Dog paperwork for travel to Bolivia

What paperwork do I need to bring my dog to Bolivia? I am applying for a 'special purpose' visa to lake care of family. I want to bring my dog. What dog paperwork should I bring to the consulate when I apply for the "Special Purpose" visa? And then what paperwork will I need when we are approved? Thank you!
