Generally, you need to have a passport that doesn't expire for at least 6 months, a yellow fever shot, a round trip ticket or copy of travel itinerary, proof of hotel reservations, a passport-sized full color photograph, bank statement or equivalent that shows economic solvency. Requirements may differ from one country to the other but these are the general requirements. The cost is different for each country too, depending on agreements signed between countries. Not sure what the cost is for Indian nationals.

Indian nationals do need a tourist visa to enter Bolivia, but do NOT need to get it in India. You can get it at the airport in Bolivia when you arrive. However, you must be sure you have complied with all the requirements. India is in the GROUP II list of countries. See more about tourist visas here.
