Sole custody of minor child traveling to Bolivia

I have court awarded sole custody of my minor children and have documentation. My ex was awarded limited visitation at the time. The youngest, 15, chose 2 years ago not to continue to see his father becasue of safety concerns but we have not had this legally ameneded. My question is this, when applying for a Visa, do both his father and I have to sign, even though I have legal sole custody, including sole decision making rights, since the father has visitation rights only?
